Non-Medicaid Service Coordination

This program offers service coordination for non-Medicaid Medically Fragile, Technology Dependent, Chronically Ill and Developmentally Disabled children. Angela's House has a Family Support Service grant with NYS Office of Mental Retardation and Developmental Disabilities to serve families who reside in Nassau and Suffolk Counties.

The service coordinator will work with the family to access eligible services and resources needed to maintain the child at home.

 

Eligibility Criteria

  • This program offers service coordination to families of Medically Fragile, Technology Dependent, Chronically Ill and Developmentally Disabled persons
  • Family must reside in Nassau or Suffolk County
  • Will help children from birth through 18 years of age
  • The family cannot be enrolled in any Medicaid program or receiving any case management / service coordination service
